To save time, we ask that you complete the enclosed registration and medical history forms and bring them with you to your appointment. You may also bring your medication list to make certain we have all of the necessary information on file.
Your initial visit will include a comprehensive examination (or Consultation), necessary x-rays (unless you are expecting), an oral cancer screening, cleaning, fluoride treatment, and oral hygiene instructions. This visit will last approximately one hour and we encourage you to ask questions.
If you have had x-rays taken/made within the last year, please bring them with you to avoid having taken/made again. Keep in mind; x-rays are required in order to do a thorough, comprehensive examination.
Please remember to bring your dental insurance card as well as a valid state issued photo ID when you come in. We can then include this information with your records. Also there is only metered parking on Spring Street and in the parking garages.
